        UCSF designates this enduring material for a maximum of 11.0 AMA PRA Category 1 Credits ™. Physicians should claim only the credit commensurate with the extent of their participation in the activity. The total credits are inclusive of 10.0 in CT and 1.0 in MR.

        Series Release: May 16, 2015
        Series Expiration: May 15, 2018 (deadline to register for credit)

        CME credit is obtained upon successful completion of a multiple-choice self-test and evaluation (included with the series), as well as an additional $45 processing fee. CME Credit registration forms must be submitted prior to series expiration date, certificates will be dated upon receipt and cannot be dated retroactively.
